Dr. Crawford has chosen dental laboratories that use high quality materials and provide excellent craftsmanship to fabricate crowns, bridges, dentures, and partials that meet his stringent standards.
Fees are due at the time of service. We accept cash, check, Visa, MasterCard, or Discover.
Our office has partnered with CareCredit to offer our patients a variety of affordable flexible payment options. Several payment plans are available; including interest free (if paid within the promotional period), no initial down payment, extended payment plans up to 48 months, with prepayment allowed anytime. We want to help you take maximum advantage of your dental insurance. We will happily file claims for you at no charge. At your request, we will submit a pre-treatment estimate for proposed dental treatment. However, our office can not guarantee payment or coverage by your insurance company. Since dental insurance normally covers only a portion of your charges, and can vary widely from one policy to the next, we urge you to be fully aware of the provisions of your own dental policy. Payment for your portion is due at the time of service. Any discrepancy between the estimated insurance portion and what is actually received will either be refunded to you, or billed to you.
We are currently a network dental provider for Aetna, Anthem, Delta Dental, United Concordia, and participate in the CIGNA Dental Network Savings Plan. Other plans are in the process of being added soon.
When you request we set aside time in the schedule for your dental treatment, we are reserving that time exclusively for you so that we may devote our attention to your dental needs. Since time with our patients is very important to us and lost time is irretrievable, we must charge for broken appointments and no notice cancellations. To avoid this fee, all we ask is that you give us a minimum 24 hours notice and we will gladly make any changes you require.
When you call with an emergency, Dr. Crawford 's main concern will be to relieve any immediate pain you are experiencing. Since often it is not known what your treatment needs will be until seen and evaluated, a subsequent appointment may be required to adequately complete necessary procedures.
If you require a specific day or time, we highly recommend that you schedule your regular dental cleaning and check up in advance. If you prefer not to schedule in advance, we have a courtesy reminder serivce and are able to notify you by email or telephone message that the time to schedule is approaching.
